To Reserve your spot now, fill in the following information and mail with
$50 deposit to the address above. Deposit is non-refundable
after April 2, 2005. Remainder of fee ( $180) is due on or before April
2, 2005. If full fees are not received by April 2, 2005, your
space may be given to an alternate on the list. Lunch is
included in the fee.
Waiver must be signed by the owner or the owner’s authorized agent, at
the time of payment, or entry is not considered valid.
Sponsors may remove any dog on account of disease, viciousness or other
cause and no one shall have any claim against the sponsors or their agents.
Entries will not be refunded in the event a dog is absent. If because of riots,
civil disturbances, or other acts beyond the control of the sponsor it is
impossible to open or complete the clinic, no refund of entries will be
made.

Tammie Rogers and Robert Rogers sponsor this event with regards
to the word sponsor below.
In consideration for participation in this event, I agree to
assume all responsibility for any claim, loss or damage, of whatever
kind or nature, whether to person or property, which may be caused
at or near this event, either directly or indirectly, by me or the
dog or dogs I have entered in or brought to this event. I further
agree that I will hold the sponsors harmless and defend them from
any and all liability for any injury, claim, damage or loss, of
whatever kind or nature, whether to person or property, caused at or
near this event, whether directly or indirectly, by me or any dog I
have entered in or brought to this event. I agree to indemnify the
sponsors for any loss, cost or expense including attorneys fees and
costs, to which the sponsors might be subject as a result of any
claim, suit, loss or damage caused in any way by any act or
negligence on my part or on the part of any dog or dogs I have
entered in or brought to this event. I certify and represent that
the dog or dogs I have entered in or brought to this event is/are
not a hazard to other dogs, other animals, or to people.
